Welcome to the Colorado Hospital Transformation Program Participant Portal. On this page, you will be able to submit your HTP Application and Implementation Plan. From this page you will also be able to access the module for your periodic activity and milestone reporting and upload any required self-reported data. You will also be able to access the HTP Dashboard, which provides an overview of your hospital's interventions and their associated milestones, as well as a summary of program funding and budgetary adjustments.
